小螺絲釘們跨界、跨域學習-來自高榮林清煌主任SP培訓課程 (撰文:蘇兆基/周則銘) 什麼是標準化病人? 很多文藻同學可能並不知道,但在2021年10月21日,有許多同學們接受了第一堂標準化病人的培訓,同時也開始了取得標準化病人證書的路程。 第一堂課的講者是高雄榮民總醫院一般內科主任,也是高雄榮民總醫院 OSCE 組織成員的林清煌主任。 這天的課程內容包括標準化病人的簡史、標準化病人在 OSCE 考試的用途以及如何取得標準化病人認證。

本次的演講讓文藻學生了解了醫生培訓過程、實習、道德原則和理解患者的重要性。 醫生接受的專業培訓是為了確保有足夠的知識和技能來做好醫院的工作。 不過,林主任表示,如果醫學院學生要想成為醫生,光靠知識是不夠的。 學生在成為醫生之前,需要熟悉12個學科領域的所有知識,不僅如此,還需要通過診斷病人的OSCE考試,才能正式成為一名醫生。 因此,標準化病人在這個考試過程裡就顯得非常重要。

標準化病人是在醫師練習問診中扮演病人角色的專業人士。林主任簡要介紹了標準化病人的歷史後,也跟學生分享為什麼需要標準化病人和他的重要性。OSCE是醫學院學生成為醫生必須通過的考試,確定醫學院學生是否具備成為醫生的適當技能和知識。如果教授認為學生不適合當醫生,他們會要求學生退出。在這樣的前提下,學生在這考試中的壓力將會很大,但他們仍必須面對這些挑戰,因為他們將來要面對的是真正的病人。 林主任說「讓 OSCE 考試對所有考生都公平的一種方法,就是每一場的表演都統一標準,這就是我們所說的標準化病人了。」「只回答醫師問你的問題,不要透露太多信息。OSCE 考試目的在於醫學院學生是否可以診斷出患者的健康狀況,如果你給出太多的提示,會幫助不合格的學生通過考試,在未來這可能會造成更多的問題。」此外,標準化病人還必須能夠像實際患者一樣表現出疼痛和情緒。這也是為什麼在標準化病人的培訓中,將會安排一堂戲劇演練課程的原因。通過適當的培訓和實踐,任何人(20 歲以上)都可以成為標準化病人並為醫療行業做出貢獻。成為標準化病人是有意義的,它可以幫助很多醫學院學生在未來為社會做出貢獻。

講座結束後,文藻同學對台灣的醫療體系有了更清晰的認識。 他們也都很期待下一堂的戲劇演練課,這將也是他們的另一個新體驗。

Becoming a Standardized Patient – How Wenzao students can play a part in the medical training of doctors.

What is Standardized Patient? Many students from Wenao may not know the answer to this question, but on October 21, 2021, the students were given their first lesson in a series of training courses to become a certified Standard Patient. The presentation was made by Ching-Huang Lin, Director of General Internal Medicine and a member of OSCE organization at Kaohsiung Veterans General Hospital. The contents of lecture include a brief history of Standard Patient, the role of a Standard Patient during OSCE, and how to become a certified Standard Patient.

This presentation helped Wenzao students learned about the importance of doctor training, practicum, moral principle, and understanding patients. The professional training that doctors receive is to ensure they have enough knowledge and skills to perform their jobs well in hospitals. However, Director Lin said that obtaining knowledge is not enough for medical students to become doctors. Before medical students become doctors, they are required to be familiar with all the knowledge in 12 subject areas, not only that, they also need to understand how to become a doctor by being able to diagnose the patients’ health. This is where Standard Patients come in.

A Standard Patient is a healthy individual playing the role of a sick patient during the doctor-patient interview. After Director Lin gave a brief history of Standard Patient, the students learned the importance of Standard Patients during OSCE, an exam that medical students have to pass in order to become doctors. During OSCE, the medical students are determined whether or not they have the appropriate skills and knowledge to become a doctor. If the professors thought that a student was unfit for this field, they would invite the student to withdraw from the program. Imagine the high pressures that medical students have during this examination, but they have to face these challenges because they will face real patients in the future. “One way to make the OSCE test fair all the examinees are for you to perform every case the same as a Standard Patient” said Director Lin. “Only respond what you were ask and don’t give out too much information. The purpose is on whether the medical students can diagnose a patient’s health issues. If you give hints that helped an unqualified student pass the exam, it can be a serious issue in the future.” Furthermore, a Standard Patient must also be able to act out the pains and emotions like that of an actual patient. This is why as part of the Stand Patient training, there will be a lesson with an acting coach. With proper training and practice, anyone (over the age of 20) can become a Standard Patient and contribute to the medical industry.

After the lecture, Wenzao students have a clear concept of the medical system in Taiwan. They all look forward to next lesson with an acting coach which will be an experience they will not have in any of their classes.
